On or about June 28, 2019, while utilizing a Privilege to Practice (PTP) nursing in Conroe, the RN provided a falsified Basic Life Support (BLS) card in that she altered the issue and expiration date. In addition, the RN provided a fraudulent immunization record that did not contain patient identification information. The RN’s certification, which was required for the position travel assignments with several companies.

The Texas Board of Nursing sent a letter to the RN regarding the issue so then she can have the chance to defend herself.

The RN states she sent over whatever she had already saved as attachments in her email because she was not at a computer at the time of the request. She states she never actually looked at what she was sending, only the titles of the saved documents. The RN states a few weeks later, Medical Staffing requested a copy of her BLS card. She states she told the lady that she had to recertify. She further states she did not intentionally give the BLS card volunteering after the hurricane. She states her wallet was damaged and the other volunteer was trying to help her out. The RN additionally states she has never given any false information on her certifications.

However, she was not able to provide a good defense for herself. Therefore, the Board placed her RN license to a disciplinary action instead.
